Please complete the following online forms for a DE player who wishes to play on a team in another state. Once the forms are complete and printed, make sure that the player signs the Registration & Release to Play outside of DYSA form and the parent signs the Player Application form. All forms must then be mailed with a copy of the player’s birth certificate, a 1” x 1” picture (head shot) and a $25 check made payable to DYSA. Mailing address is DYSA, c/o Melody McNulty, 80 Beech Hill Drive, Newark, DE 19711.

FOR PLAYERS WISHING TO TRAIN WITH ANOTHER TEAM
TOURNAMENT PLAYER PASS – This pass will be issued for non-carded players for tournament use only. Players may not apply for a Tournament Player Pass to guest in any league or train with a team. Click here to apply.
Players will need to supply the following:
- Birth Certificate (needed if player did not play in Delaware last year)
- Photo (passport size or smaller)
- All required out of state documents (player and team information completed and form signed by out of state official)
- appropriate DYSA fee
Registrars are unable to card a player with blank forms! It's the player/parent's responsibility to contact the out of state office and obtain all appropriate forms.
